Stewardship Ontario has prepared this survey form to enable you to submit your comments on the consolidated Municipal Hazardous or Special Waste Waste (MHSW) Program Consultation Document with greater ease. The form consists of a series of survey questions and the deadline to receive comments is March 16, 2009.
Please note the following:
- If your comments do not fit the suggested categories, use the “general/other” question to share your views.
- This survey also allows you the ability to upload any supporting documents or correspondence you would like to send to Stewardship Ontario.
- You can save the survey and return at any time. Click on "save and continue survey later" at the top right-hand of each page. Please make sure that your mail server can receive mail from: customerservice@stewardshipontario.ca.
- Don't use the back/forward browser buttons at the top of the page. Use the buttons at the bottom of each page to move between pages.
- When you have finished the survey, click "Finished. Submit Your Survey" to send your comments. A copy of the data will be sent via email. If you do not receive this, you have either not submitted the survey or you have an error in your email address. Please contact us if you believe that your survey is complete and you have not received a copy.
