Thank you for participating in this study. We are conducting research on how technology can play a role in enhancing more healthy lifestyles. Specifically, we'd like to understand how we could make it easier for people to find information that support their health goals, whether those goals are to lose fat, gain muscle, eat better, find an effective workout program, or similar. In short, we'd like to put everyone fewer clicks away from health and happiness!

The survey has a few sections: (i) Basic demographic information, (ii) Basic physiological information, (iii) Goals, (iv) Use of physical monitoring devices, (v) Use of the internet to help you with your health goals, and (vi) Use of other online tools and downloadable software to monitor nutrition progress.
