Welcome to the Partnership to Enhance Research Related to the Management of Chronic Disease Survey (HUM00001234)
This study looks at the challenges that people and their caregivers face when they have a chronic disease, how they cope with these challenges, and how they get information about their disease and its treatments.
This online survey contains a combination of multiple choice questions and open-ended, fill in the box types of questions. It will take 50-60 minutes to complete but can be completed in multiple shorter sessions if you provide an e-mail address that will be used to e-mail you a link so you can return to complete your survey at a later time.

As a small thank you, those who complete the survey can opt to be entered in a drawing for a $100 Visa gift card; a drawing will take place among each group of 250 participants who submit completed surveys.
All study participants must have a chronic disease and be at least 18 years of age or be an adult who helps or gives care to a person of any age with a chronic disease for some portion of time each day. If you do not wish to participate, click the “x" in the top corner of your browser to exit.
