Fall Risk Assessment
The facts about falls:
~ One third of adults over the age of 65 fall each year
~ 20-30% result of those falls result in moderate to severe injuries impacting their ability to live independently
~ 60% of those falls happen at home
~ Most of those falls are preventable
This assessment is designed to help determine if your loved one is at risk for falls at home. It should only take you a couple minutes to complete.
When you submit the assessment you will receive a Fall Risk summary that includes a list of possible measures you can take to decrease the fall risk for your loved one.
Has your loved one fallen in the last 6 months?
How would you describe their balance?
Are they able to walk without shuffling their feet?
Do they use any of the following assistive devices while at home?
Are they able to use the walker safely?
Are they able to use the wheelchair or scooter safely?
Are they able to use the walker safely?
Do shoes and slippers have a rubber sole and low heel?
1. Does your loved one have symptoms or a diagnosis of dementia or other illness that impacts memory and judgment?
2. Do they understand their own limitations?
