Through its Advisory Board, Partners and stakeholders, the Florida Office on Disability & Health (FODH) has developed a Health Condition Survey. This survey is designed to allow a caregiver or family member to respond on behalf of a person with disability. Your participation is important because information obtained will assist in understanding and focusing on issues affecting persons with disabilities and their health care in the State of Florida.
Your participation in this survey is completely voluntary. No compensation will be provided for participating in this survey. You must be 18 years or older to complete this survey. You may skip any questions you do not wish to answer and you may stop this survey at any time. The estimated time of completion for this survey is 20 minutes.
Your identity will not be known and no personal identifying information about you or the person you are telling us about will be recorded through your participation in this survey. There is no risk to you should you choose to participate in this survey. By completing and transmitting this electronic survey, you are acknowledging informed consent and that your participation is completely voluntary. There are no direct benefits to you for participating in this study.
