This survey is the start of the Recommitment process for the Moab Century! It's your way of telling us what activities you would like to participate in during your event weekend. If you are fundraising for this event (as a participant, mentor or captain), Team In Training will pay for the following expenses on your behalf:
• Your bike transport to/from Moab
• Your flight reservation
• Your portion of the hotel room (all hotel rooms are booked as double occupancy)
• Your ground transportation if you are on the Team Flight
• Your ride entry
• Your Inspiration Dinner ticket
• Your Victory Celebration ticket
• Your event jersey
This survey should take about 15-20 minutes to complete. While in the survey, you may go back and edit previous answers. Once you complete the survey, you cannot go back and change your answers, so please read the information and questions carefully.
This survey is only one of three parts of the Recommitment process. In addition to the survey, you must download the
Volunteer Recommitment Agreement,
Ride Entry, and the
Payment Form from
www.tntanswercentersf.org (under Recommitment). The Payment Form is only required if you plan to incur extra charges for the Moab event weekend.
The survey must be completed online and the Payment Form (if applicable) must be submitted to LLS by
Friday, July 17, 2009.

RECOMMITMENT AGREEMENT
By committing to raise the fundraising minimum, you are guaranteeing your place on the team. This is your pledge to continue with the program to reach your fundraising goals. The credit card or blank check you provide will not be processed unless you have not reached your participation minimum at the final fundraising deadline of
September 3, 2009.
RIDE ENTRY
Please follow these instructions as you fill out the ride entry:
• Please print clearly and in BLOCK letters.
• Do not leave any fields blank.
• Read the waiver (use a magnifying glass if necessary!) SIGN & DATE it! Your ride entry is not valid without a signature. If you are 16 or 17 years old, your parent or legal guardian also needs to sign & date it.
• DO NOT send it to ride organizers.
• Non-fundraising mentor/captains may secure their ride entry through LLS. The cost is $75.
This survey, your Ride Entry and your Recommitment Agreement must be submitted to LLS by July 17, 2009.
